Individual Signature Block

This is a form signature block to be inserted relative to a party to an agreement that is an individual. This signature block requires the signature of the individual. It requires two witnesses to the signature

1. [PARTY NAME] – Insert the defined term for the name of the individual as it appears in the document. Look at the first paragraph of the agreement to ascertain this name. For example, the company may be defined as “Consultant” “Client” “Customer” “Owner” “Employer” or other defined name.
2. [Name of Company] - Insert the exact legal name of the individual. Note that this exact legal name should also be used in the first paragraph of the Agreement.
3. [Print Name] – Full legal name of individual.
4. [Signature] – Self explanatory.
5. [Witnesses] – Agreement calls for two independent witnesses to sign. These individuals should observe the individual actually sign the agreement.
